Save the Date Request
If you don’t have all the necessary information or approvals for a multi-day event, but would like to place a non-guaranteed hold on space in the building for your desired event dates, please email Jillian at info@beth-david.org.
Once you have your information, please be sure to come back and submit an intake form.
Hosting An In-Person Event
If you are planning something either at Beth David or elsewhere and you would like it listed on the Beth David calendar and assistance with publicity, you must fill out an Event Intake.

Committee Meetings
Are you representing a Beth David committee? Are you looking to submit information for committee meetings?
If you are looking to simply request meeting space for a committee meeting that is open ONLY to the committee and not promoted in any way, go to our “Express Meeting Request” form.
Vendors / Speakers Hired Directly by Beth David for Congregational Program
All persons being paid by Beth David must enter into a Professional Services Agreement. The agreement and W-9 must be submitted for approval by the president or designee in order to receive payment. We also prefer to pay by direct deposit. Please have the Direct Deposit form completed & returned. We don’t need a voided check, just need to be sure the routing & account numbers are correct.
