Current Captain's List - A six month listing of the current captains
Action Form - The form used to collect information from the congregant in need
Call Record Form - Used to record calls made to needy congregants
Captain Summary Form - Filled out at the end of the two week captaincy
Captain's Instructions - A summary of the process for serving as a captain
Captain Action Process - an overview of the captain's responsibilities and the process

sides of one paper.
